Requests submitted through the Site (consult requests, contact forms, the VIP list, quiz submissions) are requests only and do not confirm an appointment. An appointment is not booked until we confirm it with you directly. Booking and scheduling may be handled through third-party platforms; your use of those platforms is also subject to their terms.
Whether a given treatment is appropriate for you is determined only after an in-person evaluation and, where required, a medical evaluation by a qualified provider. We may decline to treat anyone who is not an appropriate candidate.
Important: These Terms of Use cover the website only. They do not govern the treatments themselves. The terms of any treatment — including informed consent, risks, financial and cancellation policy, and any release — are provided to you separately and signed in person before treatment. These Terms are governed by the laws of the State of Arizona, without regard to conflict-of-law rules. Any dispute relating to the Site will be brought in the state or federal courts located in Pima County, Arizona.
